In
addition to post acute services that are used to manage high
cost services, FHA also provides several select services to
acute care providers. These services are designed to assist
acute care providers to manage higher cost patients in a quality
and cost effective manner.
FHA
services include:
ACE
Program Development. This program provides the environment
and care delivery model for frail elderly patients that foster
a higher quality of care, prevent further physical and mental
deterioration during the acute care stay, and enhance cost efficiency
allowing acute care providers to deliver care.
Senior
Programs. The services consist of a variety of programs
designed to create linkages between seniors and providers, foster
brand loyalty, provide support services to families and physicians
for better management of the frailer senior and promote health
through education and healthy living.
Medical
Staff planning based on specific community needs and
development.
Community
based care management systems eliminate unnecessary
emergency room visits and hospital admissions.
Neurosciences
Center of Excellence, designed to foster growth in
the inpatient and outpatient segments for this population.
Specialty
outpatient programs, such as CORFs, day hospital for
the pulmonary and congestive heart failure and Federally Qualified
Clinics.
